Prior to America’s entry into World War I, Millsap served in the 2nd Missouri Infantry, a National Guard regiment with companies stationed throughout southwest Missouri. When the regiment was activated for Federal service in 1917, it was assigned to the 35th Division, a new force organized from other Missouri and Kansas National Guard units. The 35th was sent to Camp Doniphan, Oklahoma, for training where the 2nd Missouri was converted into machine gun battalions. Upon arrival in France, the 35th Division was assigned to a relatively quiet sector in the Vosges Mountains before joining the Meuse-Argonne offensive on September 26, 1918. Millsap was wounded two days later. After fighting in the Argonne Forest for five days, the 35th was transferred to the Verdun sector.
Millsap returned to the United States in March 1919 and died in Mount Vernon in 1968.
